Change of Name
One of the following types of documents must be submitted to the Office of the Registrar:
- Driver's License
- Social Security Card
- Passport
- Legal court documentation
Date of Birth Correction
One of the following types of documents must be submitted to the Office of the Registrar:
- Birth Certificate
- Tax return (or parent tax return if applicable)
- Driver's License
Social Security Number Correction
A Social Security Card must be presented to the Office of the Registrar if there is an error.
All documentation can be submitted in person, U.S. mail, or by fax:
